THE United States has apologised after a State Department presentation used an apparently AI-generated map that incorrectly represented all six African countries highlighted during an international AIDS conference in Rio de Janeiro.
The mistake was more than an embarrassing geographical blunder. US officials were explaining health agreements that could reshape funding, data management and national responsibility across Africa when their presentation failed a basic accuracy check.
Six countries represented incorrectly
The slide was shown on July 26 during a session at the AIDS 2026 conference on new US government health agreements.
It purported to identify Nigeria, Mozambique, Malawi, Uganda, Cameroon and Cote d’Ivoire. However, none of the six countries was represented accurately.
Nigeria appeared as a vast landlocked territory stretching across the Sahara rather than a coastal West African state. Mozambique was moved away from its position in south-eastern Africa towards the Horn, while Cote d’Ivoire appeared on the eastern side of the continent.
AIDS policy specialist Emily Bass, who first published an image of the slide, said Uganda and Malawi were placed in approximately the correct regions but were given inaccurate borders. Cameroon’s label was not connected to a country.
Reuters authenticated video of the presentation and found an OpenAI watermark indicating that the graphic had been produced using the company’s tools. OpenAI said it was investigating the report.
The watermark does not establish who entered the instructions, altered the image or approved it for official use.
The Washington Post reported that every country the slide attempted to highlight was shown incorrectly. The distinction is important: the graphic did not label all 54 African countries, but all six countries it identified contained geographical errors.
State Department accepts responsibility
The State Department blamed the mistake on a team member who hurriedly altered the presentation shortly before it was delivered.
It accepted ‘full responsibility’ for the confusion and misrepresentation caused to those attending the session, including representatives of the affected African countries.
The explanation nonetheless raises questions about the safeguards surrounding government use of generative AI.
The technology can rapidly produce polished-looking maps, illustrations and charts, but visually convincing material may still contain fundamental factual errors. The failure in Rio was therefore not simply that an AI tool generated an inaccurate image. The graphic was also selected, inserted into an official presentation and shown publicly without effective human verification.
The incident comes as African policymakers and analysts press for greater continental control over artificial intelligence, digital infrastructure and data governance. An Africa Briefing analysis of Africa’s AI future argued that the continent must play a stronger role in shaping the technologies and rules increasingly influencing its public institutions.
Health funding gives error greater weight
The setting made the mistake particularly sensitive. The map appeared during a presentation about Washington’s bilateral health agreements, rather than during an informal technology demonstration.
The State Department’s transition strategy requires partner governments to assume increasing responsibility for financing and implementing HIV programmes as US assistance is progressively reduced.
Washington argues that the approach will promote sustainability, stronger national health systems and greater domestic investment. The strategy also allows American support to be reduced when governments fail to meet agreed financial or performance commitments.
However, some African governments have questioned the financing obligations, data-access provisions and wider conditions associated with the new model.
Africa Briefing has reported that new US health agreements involve significant funding reductions as partner governments are required to provide more of their own resources.
The broader decline in donor support is also forcing African health systems into a difficult reset, with the drive for greater national ownership unfolding alongside immediate risks to treatment, prevention and disease surveillance.
Ghanaian Communications Minister Samuel Nartey George has said Accra rejected a proposed US-backed arrangement over health-data sovereignty concerns. Africa Briefing previously examined Ghana’s objections to the proposed health data agreement.
Against that background, an inaccurate map risked reinforcing perceptions that Washington was restructuring important African health partnerships without sufficient attention to the individual countries involved.
A test of diplomatic credibility
The United States remains one of Africa’s most important health partners. PEPFAR has supported HIV treatment, laboratories, prevention programmes and health workers across the continent for more than two decades.
But diplomatic credibility depends on more than funding. It also requires careful knowledge of partner countries and respect for their distinct political, geographical and public health realities.
The State Department said the conference discussions remained ‘substantive and constructive’ and reaffirmed its commitment to fighting AIDS. That suggests the immediate negotiations were not derailed by the slide.
Even so, the episode offers a clear warning for governments adopting generative AI. Official maps, charts and documents require the same editorial scrutiny as material produced through conventional methods.
Correcting the presentation will address the immediate error. Preventing a repetition will require stronger verification rules and clear human accountability for AI-generated content used in diplomacy.
